In modern kernels, the Realtek driver does not have models. The analog and HDMI outputs are two separate devices; this must be configured with PulseAudio.

Sounds like a firmware bug. You might try writing a script that is called by udev when the HPs are plugged in and that sets mixer levels.

In theory, alsactl restore should be run automatically, but it appears you did not deactive PulseAudio completely.
