"Kernel driver in use: snd_intel8x0" means that the driver is loaded, and attached to the device. Your problem (whatever it is) has some other reason.

It is easy to compute the frequency from a MIDI note number, but there is no software that display this in real time because that value is usually not useful for humans. What do you need it for?

These mixer controls look OK. It's possible that the "Auto-Mute Mode" makes problems, or that your hardware makes this connection.

I guess you didn't explicitly enable loopback in PulseAudio? Run alsamixer, and look at the controls of the hardware device.
