I fought with a similar problem using ALSA. For that, it was a question of creating a "default" alsa device (pcm.!default). Perhaps something similar with an alsa loopback device would work (see: remastersys.com/forums/index.php?topic=2800.0).

I similar problems in a new system I just built with 13.10. I tried many suggestions that didn't work. Then I disabled (uninstalled) pulseaudio and went to just running Alsa and have not had problems since.

This caused errors with loading the radeon kernal module. (RS690M on a eMachines D620 laptop running 10.10). It did give me a clue. I ended up adding similar lines to xorg.conf and driconf (see answer). Thanks for the hints.
