@all I think I have found it: bugs.launchpad.net/ubuntu/+source/xorg/+bug/1559308. This bug contains a comment with a syslog attached, and that syslog contains a traceback which is identical to the traceback I have in my syslog from the morning I had an external monitor attached and saw this problem. I suspect Ubuntu bugs are partly prioritized by number of users reporting they also have the problem, so please go there, register, click the 'me too' button, and add any useful info you have. Thanks!

What is 'categories'? I don't see that word in in the app nor the pull down menus. I just have a grey panel containing the text "No application data found", as shown in the OPs question.

I guess the place to find/report this bug would be here: bugs.launchpad.net/ubuntu. There are a few that look similar (e.g. bugs.launchpad.net/ubuntu/+source/xorg/+bug/1565526), but I haven't yet found one that looks exactly like it. Am I right to think that, since our problem requires a hard power cycle (i.e. cannot ctrl-alt-f1 to get to virtual terminals) this means it isn't just an Xorg hang or crash, but that the whole kernel has crashed?

I don't know if this is useful, but I eventually tracked down my driver name, which is given by sudo lshw -C video. It says configuration: driver=i915_bpo

I have the exact same problem, in every detail, except I'm on a Lenovo Thinkpad T460. My graphics chipset is therefore Intel HD Graphics 520 Mobile, i5 6200U.

@ЕлинЙ. No worries, I've realised it only happens with an external monitor connected, so I've just unplugged it until this gets figured out.

I added some small details to my answer. The 'can't hold down a mouse button' problem only manifests with the trackpad buttons, not when left-clicking by pressing the body of the trackpad itself. To workaround the lid-close hanging, I redefined the behaviour of "lid-close" to "do nothing".

As a workaround, you can redefine the "close lid" behaviour to "do nothing", and define a keyboard shortcut to suspend/sleep etc. See my answer.

@ЕлинЙ. Encouraged by your comment, I upgraded from Ubuntu 15.10 to 16.04, to try out the fixed lid behaviour. Unfortunately this has made my T460 unusable, as it hangs and requires a hard reboot around ten seconds after logging in to the desktop. (My fault, not yours) Fingers crossed a fix for this comes pretty quickly.
