I am looking for a web-based open source software which takes user input in a series of forms and generates a document from the same (Microsoft Word preferably). It would also need to have workflow based approval functionality. The idea is to automate a document creation/review and signoff process so that human error is minimized.
I would prefer a LAMP/WAMP environment to host the same. Therefore it would be nice if there is something in PHP. A Java based application is also fine.
